Planning A Trip Forms and Facts

Are you planning a trip to Life Adventure Center? This page contains helpful documents that will help you prepare for your trip.

	The Environmental Education Program Brochure is an overview of our program.
	The EE Group Planning Document needs to be filled out prior to attending to help us plan your program.
	The EE Parent Letter describes our programs and the consent form and can be distributed to participant parents with the consent form.
	The consent form must be signed by all participants, volunteers and staff in order to participate.
	Information for the Chaperones helps volunteers prepare for the trip.
	The Pre-trip checklist is FYI to assist in planning your trip.
	The EE FAQ will answer many common questions you may have.
	The Activity Selection Menu gives a brief description of the classes that can be selected for a program.  The classes will be lead by our facilitators and their correlation to KY core content is listed next to each class.
	The Post-Trip Feedback document will help us continue to improve our programs.

Pricing Information:

$8 per child for 3 hours of Environmental Education
$15 per child for 6 hours of Environmental Education
(Pricing is based on actual instructional time.)
